Fee Schedule

Unlike larger, more traditional medical practices, Emerald City Wellness is small by design. Because of this, you can be reassured that your time will be set aside for YOU and we rarely, if ever, run late. On the other hand, when patients are late, or don’t show, it greatly effects our ability to schedule other patients. For this reason, there is a cancellation and no-show policy in place. Please carefully read the “Financial Policies” section below for a full explanation.

Teresa is not contracted with any insurance companies and does not bill insurance but will provide a “Super Bill” that many patients submit to their insurance companies for out-of-network reimbursement. Many patients choose to use flexible spending or health savings accounts to pay for services. These accounts can’t be used for food purchases.

All appointments are billed based on time, as follows:

  • Complimentary Get Acquainted appointment:

    All patients are recommended to take advantage of a short, get-acquainted appointment so we can be sure we’re a good fit for each other. This appointment can be a phone call or in person. Body composition testing is available at this appointment for a $20 fee and can be credited toward your new-patient appointment.

  • New-Patient appointment (CPT Code 99205):

    Comprehensive personal and family medical and weight history; labs ordered (or reviewed if completed within the last year); extensive discussion regarding dietary recommendations and follow-up medical requirements prior to prescribing anti-obesity medication and other medication. Allow 1.5-2 hours for this appointment.

  • Second appointment (CPT Code 99215):

    Review of results from labs and additional medical clearance, check in on questions related to dietary changes, discuss medication options in depth, and prescribe medications if appropriate. Allow 0.75-1 hour for this appointment.

  • Follow-up appointments (usually monthly) (CPT Code 99214):

    Check in with dietary and lifestyle treatments. Refill medications and/or make changes as necessary. Allow 30-45 minutes for these appointments.

  • Phone call/FaceTime (CPT Code 99441):

    Optional weekly or bi-weekly check-in between monthly appointments. Requires monthly 99214 appointment.

  • Body Composition Testing (CPT Code 0358T):

    Body Composition Testing is available as an á la carte service for people who wish to follow their body composition during weight loss and/or athletic training. I will include a brief interpretation of the results during this 10-15-minute appointment.

  • Grocery Store Experience and/or In-Home Pantry Consultation (CPT Code 97802):

    Assistance learning to navigate the grocery store and/or evaluation of potential triggers at home. Includes personalized report that may include photos, recipes, and specific written recommendations approximately one week following the consultation.
    Available to established clients only.

    $250 first hour, then $150/hour, billed in 30 minute increments + mileage.

In addition to the above mentioned appointments, there are maintenance level appointments for people who are well-established and are scheduled for shorter appointments (generally not starting until 8-12 months into treatment). These appointments do not include body composition testing but monthly testing is highly recommended during weight loss.

Additional Costs

Additional costs to you are external and include costs of prescriptions (ranging from approximately $10 to $300+ depending on medications we choose together), lab costs, and potentially an EKG to verify cardiac clearance prior to starting stimulant medications. Other medical services may be suggested based on your specific medical situation.

There are no required packaged food costs as I strongly recommend “real grocery store food.” We do carry a few protein supplement options as a convenience but these are not required.

Financial Policies

Payment in full is due at the time of your appointment. Emerald City Wellness accepts Visa, MasterCard, American Express, and checks. Services are reimbursable through flexible spending accounts and health savings accounts. Note that if you are reimbursed by an HSA/FSA account, you won’t be able to have your visit reimbursed by your insurance company.

 

Teresa is not contracted with any insurance providers but is considered an “out of network provider.” At the end of each appointment you will receive a “Super Bill” with detailed information about your visit, including CPT and Diagnosis Codes, which your insurance company will need to reimburse you for your visit. Contact your insurance company for information about how to submit this super bill for reimbursement.

 

The above fee schedule includes codes insurance companies use to determine coverage. At your “Get-Acquainted appointment” we can discuss diagnosis codes that MAY apply to your situation. If you plan to submit your bills for reimbursement, it is your responsibility to verify coverage. Emerald City Wellness will not be responsible for non-payment by insurance companies. Not all insurance companies cover the services provided at Emerald City Wellness and we recommend you verify coverage if this is important to you.

Missed Appointments, Late Cancels, and Late Arrivals

When you schedule an appointment at Emerald City Wellness, that time is set aside for you alone, never overlapping another patient. Teresa appreciates that your time is valuable and has established a schedule that respects the time you’ve made to come to her office. In order to maintain this commitment, a late-cancel and missed appointment policy has been established. If you are unable to attend your appointment, please cancel a minimum of 24 hours prior to your appointment. Patients who cancel late or do not show for their appointment will be charged 50% of the appointment fee for the first late cancellation or missed appointment and 100% of the full fee for each additional late cancellation or missed appointment. A credit card number will be maintained on file for all patients to cover such situations.

 

Additionally, if you arrive late to your appointment you will be seen for the time remaining in your appointment but charged for the time originally scheduled.

Please note: Insurance companies do not reimburse missed appointment charges and may not reimburse the charge for a shortened visit that is the result of arriving late. HSA/FSA card may not be used to pay for a missed appointment.
